TWIST MEMBERSHIP FORM

The below form must be completed for all membership sales.

An email will be sent immediately after the membership is activated so if the customer is buying as a gift they should use their email. The recipient can contact us once they receive it and we can change it over.

If the customer asks, we need their email address to send them Terms & Conditions of membership and any updates to T&Cs. Then a monthly membership email, which they are able to unsubscribe from once they’ve received T&Cs.
